The time spent between a patient and provider is indicated by provider started and provider completed on the timesheet. There is also an area labeled resident start and completed that may demonstrate when a resident or other staff not previously mentioned would make contact with a patient. Excel as well as a time and date web page was utilized to analyze the time of each area which included NP for new patient and FU for follow-up for the particular provider and day. To differentiate between morning and afternoon appointments, AM appointments were indicated by being yellow, and PM appointments were indicated by being green. The data was collected and then compared by each provider (pre intervention and intervention time period) to conclude these results. This was a significant finding for the clinic, especially with the average appointment time being 80-90 mins long. This was the stimulus for implementing the standardize discharge packet. This intervention, as stated previously, included the discharge sheet with an update medication list, the sbe form and activity form. During the discharge phase of the appointment, it was asked of the providers and nurse who participated in the discharge to practice the teach back method. The teach back method is a technique used to test the understanding of information given to a patient. Utilizing sentences such as “can you explain to me what you and the doctor talked about?”, or avoiding yes or no questions, as well as asking or giving a scenario where a patient can demonstrate their understanding will increase the compliance of that patient. The motivation behind both the teach back and discharge packets was to reduce the amounts of calls that nursing received from the nurse line about documents that were highly requested and not remembering information that was presented during the
