1. Substance is often taken in larger amounts or over a longer period than was intended.
In this case study, the client named Matthew has been drinking for over 20 years since he attended high school. Matthew described the usage rate as beginning with 3-5 beers an evening, five days a week. That is already at 15-25 beers consumed a week. As time progressed, Matthew’s usage rate continued to increase, despite multiple attempts at quitting ‘cold turkey” in the past. The last usage rate reported by Matthew was now 6 beers on weeknights and 12 beers on weekends and holidays.
2. There is a persistent desire or unsuccessful efforts to cut down or control alcohol use.
As previously discussed, Matthew shared that he has attempted to quit “cold turkey” at least two times in the last 4 years due to his wife’s stated concerns. Now, Matthew is once again attempting to quit “cold turkey,” stating that he has not utilized alcohol for three days prior to his attendance to the current session in the case study. To me, this indicates a historical and contemporary example of a persistent, cyclic, desire to quit drinking. It also examples a history of unsuccessful attempts at ending alcohol consumption on his own.
3. A great deal of time is spent in activities necessary to obtain, use, or recover from its effects.
