The patient is a 46 year old, single, Caucasian woman who lives with her daughter, son-in-law and three grandchildren. The patient was orientated times 4. The patient affect is depressed, with feelings of hopelessness and tearfulness. The patient express feelings of sadness, irritability, difficulty with sleeping and frequent thought of suicide; furthermore she states that she has persistent thoughts and feelings of worthlessness and being a burden on her family, indicating that they would be better off without her. The social worker questions the patient about any plans to commit suicide and the patient states that she’ll drink a bottle of bleach. The social worker ask about any previous attempts and the patient stated that once before she attempted to drink a cup of bleach and her daughter caught her, the patient’s daughter confirms that this is true and that the patient doesn’t have access to any bleach. The LPN-Team Lead proceeds to consult Dr. Okinedo. Dr. Okinedo’s discuss the patient with Dr. Sundaram and the social worker and states that he is preparing the patient’s admission papers.
The patient and her daughter were very resistant about going to the hospital and express feelings of betrayal by the social worker for insisting that they do go. The patient indicated remorse about sharing so much with the social worker and stated that she will not do that again and indicated that she may not come back to Central Health Care – Huntsville
