The pharmacy profession undergone massive changes in the past few years with a number of countries encouraging more patient-oriented clinical roles and having pharmacists incorporated alongside doctors, nurses and other healthcare professionals into a multidisciplinary team. Certain countries such as the United Kingdom, the United States, Canada, Australia and New Zealand are expecting their pharmacists to be vastly available to patients in the community in order to provide counseling about medications. Various terms such as ‘clinical pharmacy’, ‘pharmaceutical care’ and in the UK, even ‘medicines optimization’ are being used to aid in defining the role of pharmacists.

Clinical pharmacists are widely available in various sectors such as in the hospital to have ward rounds with physicians by monitoring patients’ responses to therapy and suggesting drug therapies. Where as in ambulatory care clinics, clinical pharmacists often responsible in determining the formularies and they may perform cost-benefit study on drugs or educate insurance companies about latest drugs in the industrial setting. Hence, team based care that have a clinical pharmacist included is one good way to help reduce some of these adverse drug events since clinical pharmacists are experts in drug dosing, side effects and therapeutic efficiency. Moreover, when clinical pharmacists are active members of the care team, they are believed to increase efficiency by providing proper medication use and dosing. They counsel patients to know and solve their problems with their medications and improve adherence. They consult primary care team members about medication-related issues. They assist new patients by reviewing their past medications history and adjusting treatment options to medication prescribing patterns of the new organization and primary care provider.
