1 University of the People Unit 5 Written Assignment Analyze the report and present a summary, in your own words, of the failures and your comments on them. A preliminary investigation of flight data showed: nominal behavior of the launcher up to H0 36 seconds; failure of the back-up Inertial Reference System followed immediately by failure of the active Inertial Reference System; swiveling into the extreme position of the nozzles of the two solid boosters and, slightly later, of the Vulcain engine, causing the launcher to veer abruptly; self- destruction of the launcher correctly triggered by rupture of the links between the solid boosters and the core stage. The origin of the failure was thus rapidly narrowed down to the flight control system…

Sadly, the astronaut most well known for spacecraft malfunction during the Mercury program, Virgil Grissom, as well as two other astronauts set for the first Apollo launch were caught in the first major tragedy of NASA’s history. On January 27, 1967, the three astronauts were performing a preflight test of their Apollo capsule when their capsule caught fire. In the hours before the fire, the astronauts encountered problems with a sour smell from the oxygen loop of Grissom’s spacesuit, unexplained high oxygen flows which triggered alarms, and worsening faulty communications between the launch pad and the control roomApollo 1. All of these items were identified and discussed before the test continued. The faulty communication even halted the test countdown for almost an hour until just before the fire itself Apollo 1.…

Self-identity is a form of individuality that has been molded by the surroundings people enclose themselves with. Human beings are constantly interpreting who they are. The human mind is a stream of thought that is constantly churning in motion, while the evolution of the conscious awareness is a lifetime process of interpreting the world around us. Sherman Alexie, a Spokane-Coeur d'Alene American novelist, exemplifies the conflict of self-identity in his novel Flight, where he seeks to reveal the value of his ancestry from several tribes and render the importance of the daily challenges Native Americans face from within their history. Sherman Alexie was born on October 7, 1966, in Spokane, Washington.…

Analysis of the Crash The National Transportation Safety Board determined that the likely cause of the crash was that both pilots were fatigued (NTSB, 2010). Both the pilot and first officers failed to respond and correct a stall during snowy weather that the airplane could not recover from. Other contributions to the accident were the flight crew’s failure to monitor airspeed in relation to the rising position of the low-speed cue, the flight crew’s failure to obey sterile cockpit procedures, the captain’s failure to successfully manage the flight, and Colgan Air’s poor procedures for airspeed selection and management during approaches in snow and icing conditions (NTSB, 2010). Pilot fatigue is a major problem in the Aviation Industry.…
