One of the most important aspects of discharge planning is making the arrangements for continuity of care, which can include follow-up appointments, referrals to out-patient therapists and services, medication plans and prescriptions, and other arrangements. A good discharge planning may be one strategy to protect against early readmission for psychiatric patients because it provides stability of the patient’s medical condition, and prepares the patients emotionally and practically cope in the community, with appropriate aftercare supports. Patients with a history of repeated admission are most at risk and require special efforts to break the readmission cycle. Understanding what helps the patients to stay out of the hospital is the purpose of this study. With Effective discharge planning there is continuity of care for individuals living with mental illness which reduces suicide and hospital readmissions rates as well as improves the patient’s social functioning and quality of life for the patient).…

The patients will benefit from the business plan as they will have increased access to the Pharmacist’s clinical expertise, leading to a clearer idea of medication regimen upon discharge and better outcomes. Nonetheless, they may be concerned about the automatization of medication dispensing and the potential for errors. They can be re-assured that the technology is not error-prone, and that there are checkpoints to assure the correct patient receives the…

According to Ruggiero, Smith, Copeland, and Boxer, before discharge, the nurse should check medications to “identify discrepancies, such as medication omission, duplication, change in frequency, change in dose, adjustments, new medications not accompanied by a prescription, or omission of core measures.” This is referred to as a discharge time out. If the nurse is not confident about medications, the nurse can ask a pharmacist to help. This discharge time out ensures that patients are discharged to home with the correct medications list (Ruggiero, Smith, Copeland, & Boxer, 2015). Success will be determined if the patient uses handrails and grab bars as needed, use an assistive device such as a walker correctly, clutter and spills from the floors, and correctly transfer while using safe transfer procedures.…

Many barriers to transition of care has led to failures and challenges during the discharge process of elderly patients. Transition of care can fail at many different levels between the preparation for discharge to when the patient has returned home. It is important to note that transition of care is a complex process that requires several key components for a successful transition. Failure of transition of care does not occur just at one point in the care, but generally at multiple points of the care. Multiple issues contribute to ineffective care transitions, including poor communication between inpatient and outpatient clinicians; medication changes during hospitalizations; inadequate patient understanding of diagnoses, medications, and follow-up needs;…

Medication Reconciliation in the Hospital Setting The transition of patients from an acute care setting to a home setting is often challenging and stressful. It can be complex for the patient to understand the instructions for discharge and, more importantly, it can be challenging and dangerous when it comes to ensuring the patient understands the medication reconciliation process. Successful transition to home is multifaceted and depends partially on an accurate and complete overview of all medications with the patient. This is an imperative safety measure across the continuum of care (Ruggiero, Smith, Copeland, Boxer, 2015).…

For example, be conscientious of the different types of medication they brought with them and be aware of the different types of medication they had previously been on. If necessary, contact their general practitioner and get a summary of their previous care.…
