John* is a 18 year-old, Anglo/Brazilian male. John is currently living in a foster home by himself, John and his stepfather were homeless before being referred to a shelter. John reports he was 17 years old when he was placed in foster care. John reported having good communication with his mother who lives in Brazil. Youth communicates with her via Facebook every day. John ’ biological father is currently incarcerated and his stepfather lives in Texas. John has no contact with his biological father and has some contact with stepfather when he is in town. John reports he will enroll in a GED program at ABC Sr. High School in two weeks

Youth has been prescribed Clonazepam (5mg) once a day for anxiety. Youth reports that he does not remember being diagnosed with bipolar disorder but was prescribed Fluoxetine (20 mg) once a day for bipolar disorder. Youth reported he stopped taking the fluoxetine because it was making him sick and making his stomach hurt. John will follow up with this therapist to address the issue. Youth also reported taking a third medication but could not remember the name. John is not on medication for his ADD. Youth has a history of suicidal ideation. John was hospitalized in July (2016) for four days after a suicide attempt. Youth was also baker acted in August (2016) for one week. John spoke to clinician about past suicidal ideations. Youth reports smoking cigarettes daily and smoking marijuana and drinking on the weekends since he was 14. Youth reports using drugs/alcohol to relieve stress. History of substance abuse by any family members was also denied during intake. Youth reports DCF involvement at the age is 17 which resulted in him being placed in a foster home. History of traumatic experiences was reported. Youth reported seeing friends and family getting shot while living in Brazil but is not currently under any …show more content…
John parents have never been able to attend any family counseling with him because they’re not in his life as he wants, His mother is still currently living in Brazil with a new family, his biological father is still in prison since he was 3 years of age. John states that the main reason his life is miserable due to the poor choices his parents have made, He sometimes wishes that his mother had aborted him. He wants to do better in his life but, he doesn’t not have a strong support system to help him get through life. Despite all the difficulty John facing he’s still thankful that he have a counseling session to attend, he feels like those are his real family.
VI. Current Interventions
Youth was assigned to continue receiving individual mental health therapy from Morning Star Center once a week. Counseling sessions are held both at home at home and school, Case manager assigned youth to have no new law violations, to stop using drugs, and to register for classes as soon as possible. John is require to see a primary doctor once a month due to his ADD and bipolar disorder. The case manager requires youth to find a job or at least volunteer somewhere, John also have to contact the case manager once a week to notify her on how he’s doing.
Youth does not have any psychological symptoms at the present time and reports no current suicidal, homicidal ideation or
