The Pittsburgh Sleep Quality Index (PSQI) After preforming The Pittsburgh Sleep Quality Index on my resident. This sleep assessment is a list of questions that the client is asked on how well he falls asleep, what negatively effects his sleep, and how much energy does he current sleep amount give him. I choose this assessment because every morning when I got him up for the day, he would mention that he was tired and did not sleep well. After preforming this assessment, I was able to realized how poor and disturbed his sleep patterns were. He scored a 4 on this assessment, meaning that he is considered to be a poor sleeper. For him, the greatest perceived disturbance to his sleep is the amount of pain that he is in. By doing this sleep quality index the provider is able to see what is affecting the clients sleep, and is therefore able to alter care in order to make sure the client has enough sleep to feel well-rested and heal properly.

Vital Signs: Temp –36.9 degrees C. O2 – 94. Pulse – 102 bpm. Apical pulse – 88 bpm. BP left ar0m – 112/ 84. Weight: 143.3 lbs Respirations – 14 breaths/min. Pain: 6.

Temperature - cold lower right leg, warm everywhere else; exceptionally warm over amputation site of left leg. Skin discomfort, tenderness, pain, swelling, and redness near amputation site. Right foot has two ulcers on superior side on second and third toe. Turgor appropriate for age. This assessment is one of the most important for my client due to the importance for monitoring for infection of his stump as many signs and symptoms become more evident. Along with this, the skin assessment on his right foot is greatly important to monitor that his ulcers do not worsen, because the ulcers similar to these that were originally on his left foot ultimately lead to his lower leg
