“The purpose of providing nutrition care is to restore the state of nutritional balance by influencing whatever factors are contributing to the imbalance or altered state of nutritional status.”1 The Nutrition Care Process (NCP) 1 is a methodical problem solving process that dietetic professionals use. As a standardized problem solving method, dietetic professionals use the NCP to analytically think and make decisions to resolve nutrition affiliated issues while providing safe, high quality nutrition care. 1 The NCP involves four interrelated steps. 1 These are: nutrition assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring and evaluation. 1 Prior to the formation of the NCP, standardized language for all of the …show more content…
1 The standardized language uses particular phrases and terms which are classified into domains and are assigned separate alphanumeric codes. 1 The groups of standardized terms include: nutrition diagnosis, nutrition assessment and nutrition intervention. 1 In the NCP model, the central core emphasizes care that individualized and patient/client centered. 1 This is in conjunction with the importance of the relationship between the client and dietetic professional. 1 The ability of the nutrition professional to build a relationship that develops trust, exhibits empathy, and provides effective communication impacts the client’s readiness for change. 1 The outermost ring of the NCP model illustrates the environmental factors that can affect whether the client has the ability to receive and benefit from the plan or nutritional intervention. 1 This should be evaluated because it may hinder the outcomes of care. 1 The professional knowledge, skills, competencies, ethics, utilization of evidence-based practice, critical thinking, interdisciplinary work, and communication represents the inner ring of the NCP model. …show more content…
1 This is the section where the nutrition care process where the PES statement is written and adequately describes the problem in the nutritional status of the patient. The etiology describes the cause or contributing risk factor for the nutrition diagnosis while the signs and symptoms describes the data used to verify and support the nutrition diagnosis. By assessing and then prioritizing the nutrition diagnosis, the dietetic professional can focus on the most urgent and important problems. 1 This provides nutrition related actions for the intervention stage. 1 The emphasis on the nutrition diagnosis step is the results or implications of underlying health conditions and diseases from the nutrition problem. 1 The terms used in the nutrition diagnosis step are classified into three domains which are intake, clinical or behavioral-environmental. 1 The intake component consists of nutrition problems related to intake of energy, nutrients, fluids and bioactive substances obtained from diet or nutrition support. 1 Terms like “inadequate” 1, “excessive” 1 or “less than optimal intake of” 1 are some examples of typical labels which illustrate the change in intake of a nutrient and/or substance. 1 In addition intake domain examples also include: suboptimal energy intake 1, inadequate energy intake, 1 inadequate fluid intake. 1 The clinical component is connected to medical 1, physical 1 or
