The priority nursing diagnosis for this patient is ineffective airway clearance r/t excessive mucus and retained secretions as evidenced by expiratory wheezes, difficulty breathing, productive cough, thick mucus, and cyanotic episodes. The patient reported a history of trouble breathing, but stated he had no diagnosis as to why. The chart indicated the patient has COPD, the misunderstanding could be due to the learning deficit. The patient was observed as having difficulty breathing upon physical activity and while talking for long periods of time. The patient was receiving an inhaled bronchodilator, unspecified within the chart. The patient was receiving 2 liters of oxygen per minute, and had an oxygen saturation of 97%. This was recorded during a period of rest, and while the patient was not talking. However, upon standing, the oxygen saturation dropped to 90%. The patient was observed as being pale, and occasionally becoming very cyanotic during periods of time where he coughed. The coughing resulted in the patient becoming cyanotic for approximate 30 to 45 seconds. The patient reported having this cough for approximate 3 weeks, and during the coughing periods he simply could not get any air. The patient had sputum that was of thick consistency and of a dark brown color. The patient displays deficient knowledge in relation to proper maintenance of hearing aids. During a 20-minute session the client will be able to identify 3 ways to perform proper hearing aid maintenance and perform a battery change correctly. Additionally, the patient will identify at least three of the following types of maintenance of hearing aids; cleaning, battery checks, listening checks, and moisture protection. It is hoped they he will also list all proper steps of a battery change and how to clean hearing aids. The patient will be educated through discussion of the types of hearing aid maintenance, steps of a battery change, and proper cleaning of a hearing aid. In relation to the Theory of Goal Attainment, I formed a relationship with the patient and transactions occurred. Through the interpersonal system portion of the theory (interaction, transaction, and communication), the patient and I formulated a plan that would help him get back to a better or original state of health. In this case, the patient’s hearing was impaired, and was greatly affecting his communication. The patient stated he did not have his hearing aids because the batteries were dead, and he relied on his daughter to perform the maintenance. However, the daughter was unavailable for a number of days, and the patient had not been wearing the hearing aids because of this.
