On 6/14/17 home visit completed for the Nuckols’ family. Upon arrival of the home, the worker was greeted by Mrs. Nuckols sat the door. Worker entered the living room and noticed Graham was in his high chair eating his supper watching cartoon. Graham was cleaned and appropriately dressed. The worker also noticed Graham was a little shy and did not want to say hi to the worker. Graham covered his face and reach out toward his mother and when the worker tried to communicate with him. Worker briefly developed a rapport with mother to assess her mental health and return home. Mrs. Nuckols appeared mentally stable however physically tied.
