One alternative solution that o would have come up was to assist motivational problems are first because the staff are the people that are most involved in the matter. If they don’t feel motivated to work, then the work place will slow down and the others will see that there is no strong structure to fallow and then more issues will rise from this. A practical strategy that I would recommend to achieve a solution can be to give incentives to those who do their job. Also maybe ask the personal what’s happening not only rely on the numbers but on their experiences like on this case some of the nurses were unsure about these forms, having someone explain maybe where to find them. Having better communication with them will help build trust so they don’t be afraid to tell you that they just don’t know how to complete the form. There was probably a leadership problem because how are you not going to do what are you supposed to fill the forms out when an error occurs and you don’t do it you are not doing your job you are not following the rules. Perhaps they don’t have a strong leader, when people don’t have a sense of control a way to do the job correctly things will fall
