The intervention for Theresa will take place in a 30 day partial hospitalization program. Theresa will attend this intensive outpatient program Monday through Friday, for six hours per day. This program will give her time to work through her addiction and establish a sober lifestyle and support network.

Before the initial treatment session, the therapist will conduct a chart review of both Theresa’s medical records to gain further insight of her past and present condition. As a part of the first treatment session, the OT will begin to establish a rapport with Theresa by gathering information to create an occupational profile during the initial interview. The three assessments that will be used are the Leisure Assessment Inventory (LAI), Canadian Occupational Performance Measure (COPM), and the Screening Brief Intervention and Referral to Treatment (SBIRT).

Assessments:

Leisure Assessment Inventory (LAI) by Barabra Hawkins, Patricia Ardovino, Nancy Brattain Rogers, Alice Foose and Nils Olsen (1997). This assessment is designed to measure the leisure behavior of adults through its four components: activity participation, preference, interest, and constraints.

Canadian Occupational Performance Measure (COPM) Mary Law, Sue Baptiste, Anne Carswell, Mary Ann McColl, Helene Polatajko, and Nancy Pollock (1990).
