Serious case reviews are considered in all cases where:
A child has sustained a potentially life threatening injury through abuse or neglect or
A child has sustained serious and permanent impairment of health or development through abuse or neglect or
Important lessons for inter-agency working could be learnt.
The government requires that every agency which had a role and responsibility for the child should contribute to the SCR by having an individual management report. The reports are then collated by an overview report author, who must be independent of any of the agencies that had involvement with the child and family.
