Performing a change-of-shift report at the patient’s bedside is a best practice and insures quality hand-off. It increases patient satisfaction, promotes patient safety, gives patients opportunity to ask questions, correct any misconceptions and be more involved in their care. Present practice the nurses at this unit is not consistent with bedside reporting. Furthermore, patients are not included in the report. Preparatory work for designing the bedside shift report process, I will examine baseline patient satisfaction scores, review the existing shift report processes, will identify potential barriers and facilitators in moving to bedside shift reports. Family members are included whose who signed consent. Further inclusion criteria for the patient participant will be length of stay between five to seven days. Family members are involved in patient’s care can be included. All the nurses who provides direct patient care.
Location:
This study will be conducted on a 30-bed inpatient rehabilitation unit at St Jude medical center. Most of the rooms are private except four semi-private rooms. On average, daily census for this unit 23 to 26 patients. Patient population includes Stroke, Spinal cord injury, Brain Injury, Chronic pain, etc. An average length of stay for these patients are from 7 to 15 days depending on the diagnosis. Depending on the patient’s census, there are five to six registered nurses (RN) works each shift and three to five certified nurse assistances (CAN).

Half of the unit will do bedside reporting and other half will do report away from bedside for next two months. A consent will be obtain from patient or family member who is involved in the care. One-half of the unit, which are 30 bed nurses, will do bedside reporting and incorporating patient into the report. On the other half nurses will do reporting away from the patient and not incorporating the patients in the shift reports. Nurses will have in-service education on benefit of bedside report process. In addition, an education of using Situation, Background, Assessment, Recommendation (SBAR) format will be provided to the nurses. To avoid report interruptions, the CNAs will be instructed to inform patients an hour before the report that bedside report will take place between 7:00 and 7:30 each shift. Additionally, patients’ needs such as bathroom and pain management should be addressed before reporting time. Further, it is essential for patients will be informed about the report process on admission by providing them with brochures containing information about the report and what to expect.
