Grant project Overview
The Green Family Foundation NeighborhoodHELP program which is a component of the Florida International University’s College of Medicine curriculum seeks the support of the David Saltman Foundation of a $100,000, to fund a transportation service for the members of the NeighborhoodHELP program. This program will provide reliable transportation services to members that will allow members to attend scheduled and walk-in appointments to our Mobile Health Center (MHC ), Community Health Clinics, and Community Food Share. The primary goal of this system is to provide transportation and assist members in receiving free primary care services which would improve their quality of health. To make community food share more accessible to those with limited transportation. To reduce usage of emergency room facilities for non-emergency health care assist in the avoidance of costly emergency room visits and hospitalizations for uninsured individuals in need of care, to reduce the incidence of serious medical problems that result from the progression of unattended medical needs and To use health education to improve health habits and reduce the incidence of serious medical problems
