The case management system is complex and involves many branches of the health care network to adjust to patients’ requirements. Exchanges between segmented departments can be difficult, so “case managers assume the role of a coordinator and act as an interface between the many different disciplines,” such as rehabilitation division in the health network (Bachmann-Mettler et al., 2011, p. 7). The case management system help brings professionals from various fields of healthcare to assess patients’ needs and organized their next plan of care. It is the collaboration between the different fields that brings the best possible care to a patient in the system of case management.
