Circumstance: CSP made face to face contact with MHS and Liam to monitor Liam’s well-being.
Action: CSP and MHS discussed Liam’s recent medical appointment. MHS explained Liam received three vaccines at his last appointment. Liam responded to CSP voice, laughed and smiled. MHS continues to freed Liam baby food and increase his water intake. There were changes in his medication as noted on the MUSC summary. Liam was chewing on his tongue. MHS thinks Liam is teething. CSP observed Liam playing with his foot, lifting his head and holding his bottle. Liam continues to receive community services. MHS and CSP reviewed paperwork from medical physicians and community services.
Plan: CSP will continue to ensure Liam’s well-being and safety.
