Yesterday I attended at team meeting for Ariel. In attendance was Dr. David Muzlakovich, Sue Becker, Amy DeWitt, Jan Segar and Lowell Seyburn.

Before I summarize the meeting, I would just like to I want to thank you for the outstanding job you do on a day in and day out basis. The work that you are doing is difficult and challenging. You are doing it all very independently and professionally. It was acknowledged that the staff now present are so much better than the previous staff and you all should take a great deal of personal pride in knowing that.

On to the meeting....

Items of note:

1. The dumpster is scheduled to be picked up today, 9/28/16. Cue Ariel to make a last sweep of the home, garage and property.

2. There was an agreement reached regarding the repairs on Ariel's home! There has been a contractor assigned to make the repairs on Ariel's home. Lowell (or the office) should be made aware when the contractor makes contact with Ariel. Please note that the scope of the

There was a great deal of discussion regarding Ariel's recent behaviors. We must reinforce to Ariel that M & M Home Care staff are NOT allowed or permitted to be responsible for the children. The children should not be left unattended by Ariel. Ariel should be informed that staff are being directed to now document when the children are left unattended, the length of time of each event and what Ariel is doing that lead to the incident. An example of such a situation would be if Ariel decides to cut the lawn and the children are left without proper supervision. Cue Ariel that the children should not be left alone. Reminder her that M & M Home Care staff are NOT allowed or permitted to be responsible for the children. Promote the idea that a 10 y/o should not be left to supervise and care for 2 other children. This is just one example. Another might be her "alone time" in the garage... This too is a situation where we need to cue her on better parenting
