Mr. James is back for this third session where we will be reviewing the goals that we tentatively discussed during the last session and to finalize them in order to move forward with our tasks. We will determine if the goals that we recently talked about are sufficient or if we need to add more in order to paint the big picture to the Judge. The more classes that are taken, the better the chance of reunification; however, if provider reports do not suggest that there is a behavior change within the client, the tasks will be obsolete. There must be a behavior changed identified within the client for the department to recommend the reunification.
Self-Determination

Footman will facilitate the attainment of the goals of Mr. James. She will ensure that the goals are realistic and attainable for the client. The client and I discussed how feasible the goals would be to attain and we both agreed that they should be reasonable easy to complete …show more content…
James and I will review the informed consent agreement again to ensure that he understands the process and guidelines. Mrs. Footman will remind the client that things are confidential until behaviors are present that may cause danger to him or others.
Task-Centered Model
Mrs. Footman will be using the task-centered model with the client. We will use this model to attain the goals so that they are clearly identified and listed so that my client understands the path in which we are talking to ensure that all goals are met. By using this method, it will ensure that we are able to get a clear view of our progress throughout this process.
Tasks Implementation Sequence
Mr. James and I will discuss timelines for each task to ensure that he is in line with the required 12 month permanency goals. If there are goals that may be out of reach for my client, we will revisit the goal and incorporate techniques to help us achieve the goal. There will be a detailed table that outlines each task with the expected timetable in which the task should be completed.
