D-Met with the patient upon his arrival. This writer addressed with the patient about TEAM's decision of not reinstating him for treatment due to his non-compliance of his multiple AWOLs and following through with Hartford Dispensary with regards to making his payment to the clinic. According to the patient, he did everything on his end with the Hartford Dispensary, but the clinic wants him to be taper off on his methadone. This writer questioned the patient if he had obtained a letter from his prescriber stipulating that he cannot get off on his benzos script and no response to this question. When the patient was questioned as to whether or not he did indeed paid the Hartford Dispensary, he says, " I did, but if I have to do it again, I will." …show more content…
Please note, the patient started to fidget in his chair and says, " Well, I...I will get the money from my mother and pay them and also look for the receipt too." Then this writer addressed the payments to this clinic of which the patient still has not made payments to HCRC of the $95 payment of which the patient agreed to pay for last week and this week. The patient then says, " I thought I would have the money." This writer again reiterate her comment about his non-compliance with treatment, referring to daily dosing and following up with this writer about the Hartford Dispensary on a consistent basis. Because of non-compliance, his reinstatement is denied. However, this writer discussed with the patient about the Apt. Foundation of which the patient is willing to consider to seek admission due to the clinic acceptance of benzodiazepine and affordable weekly fees upon speaking with an admission representative. The patient is aware of the clinic's intake dates and time as he plans to go tomorrow and once this writer received confirmation, this writer will send his record to assist with
