According to the Center for Disease Control, it is estimated that there are about 6 million Americans who are affected by Gout. Gout is a form of inflammatory arthritis and can occur in the body when there is too much uric acid in the blood stream (Center for Disease Control, 2016). Uric acid can begin to form when high amounts of purines found in food begin to breakdown and form crystals of uric acid that start to build up in the connective tissues and joint space, this condition is also known as hyperuricemia (CDC, 2016). Because uric acid levels are associated with metabolic syndrome and Ralph has high cholesterol, blood pressure and glucose it is recommended that he lose some weight to help reduce some of his gout symptoms. Based on Ralph’s height and weight it is estimated that he consumes at total of 2,457 kcal/day. In order for Ralph to lose close to a pound a week it is recommended that he decrease his calorie intake to about 2,000 kcal/day, see Appendix B for detailed meal plan. Ralph’s meal plan will include foods that are high in whole grains, fiber, vegetables, fruits, and lean meats. According to his Daily Recommended Intake, Ralph should be consuming a total of 276-399g of carbohydrates, 71g of protein, 55-96g of fat and his sodium intake should not exceed 2.3g per day. During the initial assessment Ralph stated he was not active his daily activity was limited to his work only. It is recommended that Ralph participate in some form of physical activity each day for about 30 minutes. Because Ralph has not been active for so long I am recommending he walk at least 15 minutes in the morning around his home and 15 minutes in the evening. At the initial assessment with Ralph he was asked involved he was in the preparation of his foods. Ralph expressed having very little involvement he stated he eats what his wife prepares for him. He also expressed very little knowledge in knowing what foods he should be
