Hamidi and Delbahari (2011) explains the SWOT analysis as a participatory model used by groups for examining conditions and developing appropriate strategies and plans, through assessing attributes and proficiencies to foster evidence for change within a group. SWOT analysis tool analyzes internal and external environments to make systematic approach to making decisions through analysis of strengths, weaknesses, opportunities, and threats. Opportunities can foster the attainment of an organizations’ objectives (Marquis and Huston, 2015). Hiring from outside the CHWLB organization to add a CDE to the team is extremely beneficial to the quality of the DM education. “A Certified Diabetes Educator® (CDE®) is a health professional who possesses comprehensive knowledge of and experience in diabetes management, prediabetes, and diabetes prevention” (National Certified Board of Diabetes Educators, 2016, para 1). A CDE® educates and promotes understanding self-management to attain individualized behavioral and treatment targets that improve health results (National Certified Board of Diabetes Educators, 2016). Another positive opportunity is approximately 70% of the patients seen at MCWHLB for DM have California Children’s Services (CCS) insurance, which is willing to pay for the DEC visits once a Service Authorization Request is obtained.
Two Threats
Threats are the external conditions that can impend the accomplishment of the organizations’ objectives (Marquis and Huston, 2015). Two significant threats have the potential to reduce the outcomes of the strategic plan. First of all, non-compliance of DM patients at home, school, and community can cause an increase in readmission rates, A1C, and require an increase number of visits to the education center. The second threat is the high number of patients with low socioeconomic status that may have a difficult time gaining access to the clinic due to lack of transportation.
