Once stabilized to the point that she no longer needed hospitalization, she was transferred to a SNF for rehabilitation. Following rehab, arrangements were made amongst family members and a case manager to transition Heidi into a nursing home to assist with ADLs and IADLs because she was too severely disabled to return home. Upon significant improvement, Heidi was transitioned back home and had continued care with a home health agency providing physical therapy and a homemaker. All of these services were very well coordinated and supported Heidi’s transition to self care.…
Pa made it very clear at the initial interview that she has no need for personal support at this time and her main goal is to get permanent housing with supportive service to help her live independently again. Nevertheless long term care support is not what she desires. SC explains to Pa that she will be close to the Waiver program. SC informed PA that if she wishes to receive service from the Waiver program in the future, she would have to reapply from to PCA or Area Agency on Aging based on county she may be living in. The tasks performed by Home Care Workers to assist a client with daily living activities are documented in procedure manuals that have been endorsed by the (Clinical Practice Review Team) which forms part of the Australian Unity Clinical Governance Framework. Those procedures cover basic care support tasks such as assisting with showering or bathing, basic toileting practices, dressing, grooming, oral hygiene, medication assistance, basic assistance with feeding, skin integrity and care. All staff are trained and educated in the enablement model of care where they are to encourage the client to focus on their capabilities and not deficits. Since becoming the Senior Social Worker / Acting Caregiver Support Coordinator, I have provided feedback and recommendations that have directly influenced the program’s policies and procedures. For example, when I began working with the program in May 2015, Veterans and their Caregivers had 6 months to appeal the outcome of their assessment for the Comprehensive Assistance for Family Caregivers Programs. Working collaboratively with other program staff, we immediately recognized the length of time needed to be shortened.
