Waldrop stated that his teamster crew was instructed to unload materials from a box truck. The load was “stuck” and Waldrop climbed up between the load and the wall of the truck, placing his feet against the wall to push. The load “broke free and rolled away quickly” which caused Waldrop to “slide/fall” to the floor of the truck. Waldrop landed with his weight on his right shoulder and side of torso, knocking the wind out of him. Waldrop stated that he then went outside to “catch my breath”. While leaning against some crates Waldrop “blacked out” collapsed and struck his face on the ground. Waldrop stated he “banged his head” and, while he says he did not lose consciousness, Waldrop did state he was disoriented when he “came to”.
