The patient is a 59 year old female who presented to the ED from Brookestone tonight for aggressive behavior. Per documentation the patient was hitting other residents at the facility earlier during the day of 3/1/16. The patient has not shown any aggression since arrival at this hospital. No suicidal behaviors, homicidal ideation, or symptoms of psychosis was noted from MCM team member who responded to facility.

During the time of the assessment the patient would not effective communicate with this clinician. Patient did not appear to be responding to any internal stimuli. Per documentation the patient has reportedly diagnosed with Moderate MR. This clinician collected collateral information from Brookstone and the patient's brother to figure
