Summary Of Isaiah's Ability To Make Healthy Decisions

Focus: Isaiah’s family will learn, practice and demonstrate healthy decision making strategies to increase Isaiah’s ability to make healthy decisions. Ms. Smalls (MHP) and Mrs. Blakeney (MHS) review and sign Individual Plan of Care (IPOC) summary.
Intervention: MHP, MHS and Isaiah review the IPOC summary and sign the plan. MHP answer questions and encourage Isaiah to demonstrate more consistency with behaviors.
Response: MHS express agreeing with the IPOC summary and ask question for more information. Isaiah express wanting to sign the IPOC to meet his girlfriend and family for dinner. Isaiah ask to discuss gal at the next visit.
Progress: Isaiah is making minimal progress towards this goal. MHS and Isaiah sign the IPOC summary. MHS
