D/A: Consumer continues to live and receive 24hrs supervised care at the Connection Mental Health Residential Treatment Program located at 2197 Still Road, Camden Delaware, where he currently shows bedroom and pays $ 742 for room and board. Consumer has not complained about his living and housing arrangement at Still Road.
P: Case Manager will continue to monitor consumer’s current housing arrangement and report any concerns to the residential manager
B. Social Support
D/A: Mr. Jewell has limited social support. His major form of support comes from his brother Eddie Jewell, housemate, Dr. Capiro and Nurse. However, Mr. has not being in touch with brother for long time and has not shown any interest in reaching out to his brother as of now.
