SC, Jennifer Stoker, met face to face with Jonathan Harris at his home in Dallas, TX to monitor his services. SC was welcome by staff, Rita . SC observed Jonathan watching TV. Jonathan welcome staff. Jonathan was happy and seem to be good mood .The following items were discussed:
Service: Supervised Living- Jonathan would like to have help getting ready for his day
Jonathan reported staff assist him getting to start his day. He noted they make sure that he is the first one the assist in the morning because he don’t take as much time as his other housemate. Jonathan is making progress because staff is helping him get ready for the day
Adaptive Aide - Jonathan want like less back pain
Jonathan reported he has less back pain. He has a bed
