In my opinion, protecting clients’ privacy is ethically more important. If I am a client and I'm aware that my information will not be protected, I will most likely not provide the correct information particularly, those personal questions on the questionnaire.

I would say yes providers should share data with each other but only the needed information required to help the client. If it not needed then the other providers should not know about it. Also, the client should be aware of the information that is shared. Just like with my hospital patient, if the doctor needs their medical history/information, the patient signs a form that allows the doctor to have access to some or all the medical information.

Yes, they do have the right to be selective about the questions asked. If the information is too uncomfortable for the client or if it not needed to provide needed services to the client then it shouldn't be asked.
