Summary: Bi-Weekly ILP Review

LATE ENTRY
On 2/14/2017, CM met with the client to complete Bi-Weekly ILP Review. Client was dressed appropriately for the weather. In the meeting CM observed that the client was upset, sad, frustrated. CM inquires what happening. Client replies “she is having bad migraine headache for the past few days.” CM also inquires if the client met with onsite medical staff or if she went to the emergency room. Client replies in a hostile tone of voice “for what is just a migraine headache, and the only thing that can help her is Botox injections which due to her immigration status financially she cannot afford.” Then client continues to relates living in the shelter she encounter a lot of hostility with the residents; especially with Majorie Hall

Client reported she asked Administration if the was going to swapped her to her dorm where there are people like her as she quotes “intelligent, quiet and not crazy”. Client continues to report she doesn’t want to be among crazy people, and she asks GOD every day, as she quotes; “ I want to be dead. Please God kill me”. CM shoots Social Service Supervisor an alert email notifying client situation. Social Services Supervisor met with the client to conduct an assessment. Client continues to report to Social Service Supervisor as she quote “every night she asked GOD please kill me.” Social Service Supervisor while client was still sitting with CM for face to face meeting client 911 for client to be assesses. Social Service Supervisor also mention to the client for safety she will request for client to be swapped to another dorm. Client replies “no”, please don’t swapped my dorm. She continues to mention “the only way she will gladly accept to be swapped to another dorm if she will be place in a Dorm with “white people, intelligent, with no mental or drug problem.” CM reiterate to the client you want to be swapped to a predominant white dorm, and client make the correction by saying as she quote “ I want to be put in a dorm with people like me intelligent, quiet and with no substance or
