This 23-year-old claimant is filing a concurrent claim alleging disability due to diabetes, major depression, anxiety, and seizures as of 05/01/2016.
The evidence in file shows the claimant has a history of depression and multiple admissions. She was admitted for a suicide attempt 4/24/16. She had left home and moved to another state to live with a man she met online. She was found with a suicide note attached to her. She was re-admitted for another suicide attempt on 5/15/17. At the recent consultative exam she had a vaguely distressed mood, goal oriented thinking, and a flat affect. Her attention appeared distracted. The examiner opined she was preoccupied with men she met online and it was unclear if she was taking her medication as prescribed.
The DDS determined listing 12.08 was met as of 5/1/2016. The evidence in file supports this determination, however, an earlier onset date maybe warranted. The claimant was admitted 4/24/16 for a …show more content…
An earlier onset date is possible if the claimant was not engaging in SGA. Claimant meets medical disability requirements as early as 4/1/2016. However, there is a work issue. Please contact the claimant to resolve the discrepancy. On the SSA 3368, the claimant indicated she worked at the 7-11 store 2/2016 through 5/2016. She worked 20 hours per day 6 hours per week and earned 9.50 per hour. While it is highly unlikely she worked 20 hours per day, these earnings do exceed the SGA level. The claimant’s onset date is listed as 5/1/2016. When did the claimant stop working at the SGA level? Please resolve the issue. Please complete a SSA 821/823 with your findings.
