The patient is a 48 year old male who presented to the hospital with suicidal ideations with a plan and homicidal ideations towards community members. The patient reports auditory hallucinations of his stepfather telling him to harm himself. The patient reports that he does not feel safe by himself.

During the time of assessment the patient was awake, alert, and cooperative. The patient reports that he was recently release from Sandhills Regional Hospital and backpack was stolen from him. The patient reports he was unable to take his medications and is having suicidal thoughts. The patient reports a plan to cut his thought with a broken glass bottle.
