Presenting Problem: Pt is a 11yo male that was brought to NNBHC accompanied by his parents after speaking with the crisis team about suicidal ideation. Pt reports of being in an altercation with his father last night, where he kicked him in his gentials and stated he was trying to “kill him”. Pt states he does not know the what precursors taken placed before the event. Pt states SI with a plan to “jump off a roof or use a butcher knife to stab himself in the chest”. Pt states that he use to hear voices when he was sleeping, however he now hears the voices when he is awake. Pt states currently experience audio hallucinations today, denies visual hallucinations. Pt states the audio hallucinations command him to harm himself but not others. Pt states presently he does not want to kill or harm his father. Pt states early today he had thoughts of HI towards father, no accompany reasoning why. Pt states he attempted to cut his thumb off yesterday as a suicidal attempt with scissors but was stopped by his parents. …show more content…
