Ms. Waddell is a 34 year old female who presented to the ED with suicidal ideation with a plan to overdose. It should be noted that Ms. Waddell was discharge from Holly Hill today from being seen 10/3/16 for an attempted overdose.

At the time of the assessment Ms. Waddell reports she is currently having thoughts of harming herself. She expressed feelings of depression as stressor contributing to her distress. She describes feelings as: feelings of hopelessnes, worthlessness, guilt, isolation, insomnia, tearfulness, and loss in usual pleasures. Ms. Waddell has a history of chronic mental health and substance abuse issues. She states, "I don't know what to do now that I'm out, when I was home I was okay but then I got depressed and want to kill myself." According to Ms. Waddell she having conflicting thought of her

She expressed recent mood swings of suicidal ideation and just wanting to get help with her issues. Ms. Waddell reports attending several inpatient hospitalizations and substance abuse treatment programs. She believe over the course of her multiple hospitalizations there has been some progress She has a history of multiple suicide attempts. She reports compliance with medication and having an upcoming appointment with DayMark Recovery Services 10/19/16. Ms. Waddell has a history of Bipolar. Ms. Waddell reports today before she left Holly Hill she still felt suicidal. However, She reports when she got home around 3pm her mood change. Ms. Waddell expressed feelings of happiness seeing her children, however soon after her mood change to feelings of depression. Ms. Waddell noted she wants to attend outpatient
