Ms. Gore is a 36 year old female who presented to the ED via LEO under a custody order to be evaluated for suicidal ideation with a plan to overdose and hallucinations. Ms. Gore denies suicidal ideation to nursing staff. She states, "I'm not suicidal, just don't want to live anymore." Per documentation from MCM he reported Ms. Gore reported to him she misses her brother who died around this time of year. She reports to him she needs to go to a hospital to be put on the right medication. At the time of the assessment Ms. Gore appear guarded, however calm and some what cooperative. She currently denies suicidal ideation, homicidal ideation, and symptoms of psychosis. She reports a history of Major Depressive Disorder, PTSD, and anxiety. Gore mother and Grand mother was contacted. The mother reports since her son has died Ms. Gore has attempted 6-7 time to harm herself. She reports in the past she has tried to cut her wrist in 2013, overdose 4 months ago, hang herself, and done other things on multiple occasions. The mother reports her daughter has loss a fiance to cancer, found her brother dead on her birthday, and loss custody of her childrenm recently. The grandmother who Ms. Gore lives with reports her granddaughter has been mentioning not wanting to live more often recently. She states, "She (referring to Ms. Gore) told me yesterday she was going in the woods to find a place to die, so I called mobile crisis." The grandmother reports last week Ms. Gore told her she saw little men in her room. In addition, The grandmother reports when she attempted to take her to Thomasville, Ms Gore told her she felt like someone was going to shoot her. The grandmother reports managing Ms. Gore medications and giving her 1 pill around 1 pm from the bottle of 30. The grandmother reports Ms. Gore tried to take more when she refused to go into the hospital and was on her way
