Suicidal Assessment

The patient is a 24 year old male who presented to the ED via LEO under IVC until further evaluation, with suicidal thoughts with a plan to hang himself. Further, patient reports homicidal statements with a plan to several family members. The patient reports relational issues and family conflict as contributing stressors. The patient arrived with a BAC of .18. The patient denies suicidal ideations, homicidal ideations, and symptoms of psychosis. Patient denies depressive symptoms.

During the time of the assessment the patient is clam and 4X oriented. Th patient reports a history of bipolar disorder. Patient reports no current medications. The patient reports his wife and him have been separated for 4 months and for the past 2 days he has moved

The mother reports reports the patient verbally spoke to her about suicidal thoughts with a plan to hang himself. The mother reports having multiple conversations with her son about him making statements about suicide over the past 4 month. The mother states, " He drinking more days than not lately." The mother reports the patient ad made verbal statements to put a "contract on his wife" to kill her.

The father reports that he went to the patient home because his wife left in fear of he safety and her children. The father reports he tried to talk to his son about his alcohol problem, however his son became aggressive with him. The father reports the patient has made homicidal statements towards the patient wife.

The patient wife reports this morning the patient woke up drinking vodka and later drinking a 5th of whiskey. The wife reports homicidal statement were mad towards her with a plan to stab her. The wife reports the patient sent her multiple text messages to her in regards to the arguments and situation. The patient denies these messages and reports he did not send messages to his wife, however however the wife forward some of the messages back to the patient phone. The patient reports deleting messages and then reports he does not know anything in regards to messages. The wife also reports that the patient has made suicidal statements about a plan to hang self. The wife reports his trigger in their argument was when she spoke on the subject of his drinking and taking care of his 3 "sick'
