Substance Abuse Intake Form Analysis

Intake forms are crucial when working with clients in any field, but especially in the field of mental health and substance abuse. It is often the first contact that a possible client may have with the organization. Therefore it should be clear and easy to understand as a way not to overwhelm the potential client. It should also be versatile in that it can accommodate individuals who may not all speak the same language, yet ask enough questions for the professional to have a decent impression of the potential client. The intake form I chose was from WPCS: Western Psychological and Counseling Services, P.C. This facility provides a extensive list of services, focusing on outpatient counseling with individuals, family and group therapy. They also have programs that address chemical dependency, services within schools, health psychology, and the Latino Community. Seeming like an inclusive service, I would hope that their intake form reflects this in it’s ability to provide to the professional and overview of what …show more content…
Is there enough space to write what is needed in each answer space? Why or why not?
Given that most of the questions are formatted to where you check in a box I believe there is enough space to answer the questions. However where the intake form lacks in terms of space is where it asks you yes or no questions, and then wants you to go into further detail about your answer. An example of one of these questions is “Have you ever had thoughts, made statements, or attempted to hurt yourself? If yes, please describe”
Should the intake be re-formatted so it is easier to read? Why?
I believe that the intake form could be formated so all of the questions were not so compacted together. When looking at the intake form I am slightly overwhelmed by the amount of questions per page. For someone who may be struggling this could be too much for them to handle and it could veer them away from receiving help.
What would you include? What would you remove?
