The client is a Caucasian male was born Bucyrus, Ohio, and raised in Forrest, Ohio primarily by his mother and step-father. He described their relationship as "great" and that they tell each other everything. He described his relationship with his strep-father as "satisfactory." According to the client, his mother and biological father, divorced when he was two years of age. He stated his father came back into their lives when he was 9 and they remain in contact with each other. He considers his biological father more of a friend than a father. He stated he and his biological father smoked marijuana together. The client stated he is in currently in a relationship with his girlfriend of 10 years and noted they have two children together. He reported he is not satisfied with their relationship because he has to lie to her, but does it for the sake of the kids. …show more content…
He also admitted to using alcohol 2-3 times per year. His drug of choice is heroin and noted he used 1-3 bags a day. Per the client, he has a history of going to substance abuse treatments, but usually relapse upon completion.The client reported he has attended several substance abuse treatments in the past. His most recent was February, 2016 but later AMA because he had to stay longer than expected. He reported he received treatment at 2 other substance abuse facilities, but noted he successfully completed them. He denied ever attending any AA or NA in the past.
