Substance Abuse Case Study

The applicant is a 28 year old, single Caucasian female, who resides on her own. The patient resides in a two bedroom apartment in the hopes of her children returning to her care once the addiction resolve her addiction. The applicant's drug of choice is heroin and her last use was yesterday, 2 grams of heroin by IV. In addition, the applicant self-admits to also relapsing with the use of cocaine yesteday-$50 bag by IV. The applicant first began using heroin at the age of 14, but consistently been using it daily for over 3 years. As for the cocaine, her first use was at the age of 22 but less than once a month.

The applicant was a poor historian while addressing her history of substance abuse treatment, referring to detox. At one point, the applicant would provide a name but was uncertain. The applicant then google on her phone and regain recollection of

The applicant currently attends Stone Academy in East Hartford for LPN. Pertaining to her employment status-the applicant currently support herself with her unemployment check. Based on what the applicant reports to this writer, she was suspended as a residential aide for abuse to her disable client, but claims that the allegation is false because the this disable client has always been problematic and make false statement. Because of the allegation, the patient is suspended depending on the investigation from the State of Connecticut. In the meantime, as mentioned that the applicant support herself with her unemployment checks as reported to the Dept. of Labor that she was lay off. No current legal action and no history of criminal
