3. Critical Events (see Timeline): The client reported that her critical events involved her birthdays. She went to Chucky cheese on her 4th birthday, went to Great Wolf Lodge on her 6th birthday, went to Soaring Eagle Water Park on her 7th birthday, and on her 8th birthday, she went to Skyzone. She explains that she thinks her birthdays are her favorite time because her parents will take her places. She explained that she got her dog Buddy at age 3.
4. Sexual: P.C is a female. She claims to have a crush a male the same age. She has said that she hasn’t hugged or held hands with her crush yet. She has not reported any kissing either.
5. Substance Use: P.C has reported that she is taking Adderall. She has also mentioned that she has coffee

Physical Functioning/Medical Issues/Medications: The client is taking Adderall for her ADHD to help her focus in school. She has not reported any abnormal physical functioning. Her school had on file that she has passed her hearing test, but did not pass her vision test in 2010.
7. Legal: When asked about legal involvement, the client reported that she has not been involved in the legal system. It was asked about parent custody and she explained that she lives with both her Mom and Dad in the same house.
8. Educational Background, School Performance: P.C. is a full-time student at Riverview Elementary in Big Rapids, Michigan. She has reported to enjoy school. Her favorite subject is reading. She is currently taking a math, science, social studies, and reading class. She reports that she has good grades. She is involved in sports such as volleyball, which she explains she really enjoys it.
9. Employment: As mentioned above, the client is a full-time student at Riverview Elementary in Big Rapids, Michigan.
10. Recreational: Client reports that she likes to play at home in her room, plays pool with her Grandpa, plays card games with her Grandpa and Mom, and loves to make crafts. She also enjoys being outside with her
