The patient is 76-year-old patient referred directly from the Alaris nursing home where the patient has been there for subacute rehabilitation. The patient was found to have a profound anemia with a hemoglobin of 5.5. The patient was weak and unable to fully participate in any type of rehabilitation. The patient has been there since the middle of November being treated for an osteo of the right foot. The patient is known to have hypertension, diabetes, hypercholesterolemia, GERD and some history of congestive heart failure. The case was discussed with Dr. Fred Reverod who feels that who feels that because of the patient's profound weakness and inability to participate in rehabilitation and that it was imperative that the patient be immediately
