The objective of this guide line was to provide evidence based advice on the care of patients older than sixteen years who have had a stroke with ongoing impairment, activity and participation limitation. It also provided a clinical and social care guideline on the long term rehabilitation to support the stroke patients. Stroke is a leading cause of disability and by implementing effective rehabilitation intervention can improve the recovery process and minimize the functional disability. Improved functional outcome can contribute to patient satisfaction and reduce the costly long term care expenditure. A well-organized multidisciplinary team approach in stroke care can provide better outcomes. A better algorithm and implementation of these guideline can bring better outcomes through appropriate methods. The Advanced Practice Nurse (APN) can utilize these guidelines when providing care to the patient. These guidelines will guide the APNs to identify the best interventions and care plan based on the individual needs and functional
