Developing a single patient engagement platform across all care settings, with predictive healthcare management tools, will help empower patients to better manage their health and coordinate with all of their providers for improved healthcare outcomes. Although the healthcare organization may have an existing patient portal, this may not be offering the correct tools and benefits to the patient. Research previously discussed in this paper determined there would be positive outcomes of implementing a community-wide patient portal, which includes patient engagement solutions, patient education, analytics and a communication platform. Today, more people are managing the care of a family member or loved one. For these individuals, it is difficult to piece together the fragmented medical history and manage the many providers that care for them. A single platform can help consolidate providers and the care team into a single program that offers the caregiver one online location for the management of their dependent(s). A national survey by the Pew Research Center’s Internet & American Life Project, found that almost four in ten adults (39%) are caring for an adult or child with significant health issues (Worden, 2014). This illustrates the importance of the caregiver’s role in healthcare. Surveys will were also be conducted on a patient’s caregivers to judge the current effectiveness of a hospital’s patient engagement efforts and what changes are needed to support all members of a patients care team. Diabetes patient survey questions: When analyzing the patient questions, it is important to understand the limitations of an EHR based patient portal in contrast to a community-wide portal. A patient who responds by pointing to limited clinical information or issues communicating with all of their providers, or those patients that would need to monitor multiple portals would better be served by the community approach. In regards to caregivers, the more individual patients they are responsible for, the more important it is for all of the information to be in a single place.

Questionnaires that internal stakeholders take will also be used to measure the impact patient engagement has on patent compliance, outcomes and communication from the perspective of each care coordinator and outreach coordinator. This survey will be web-based using a free survey tool, SurveyMonkey.
