PURPOSE
Stone Lotus Hospital is dedicated to maintaining an established workplace to ensure efficient and quality care. Excessive absences and tardiness not only affects patient care, but also workplace morale and productivity.
This policy’s purpose is to define the standards and guidelines pertaining to attendance, the necessary communication, and the corrective action to manage attendance. Employees are required to report to work at the employee’s designated time and work station. Repetitive tardiness or absences without previous permission or notification will jeopardize an employee's appraisal and/or will result in termination.
DEFINITIONS
If an employee has an unforeseen absence due to an illness, it is required that the employee obtains a physician’s statement. If the employee occurs consecutive absences for the same illness and/or reason, the absence will be considered as one occurrence under excused or

o Excused: An absence that was approved in advance by the supervisor. o Un-Excused: An absence that has no prior approval by the supervisor.
 If an employee has missed more than 50% of the scheduled shift or clocks in after one (1) hour after the shift starts and/or leaving one (1) hour before the shift ends, it will be deemed as un-excused absence if there is no approval by the supervisor.

 Occurrence: refers to an un-excused absence(s) for the designated shift and/or consecutive shifts.  Tardy: failure to report for duty at the designated work station and shift including late returns from lunch and/or break. o If an employee is more than ten (10) minutes past the scheduled start time, has any other unscheduled non-approved breaks, late returns from lunches/breaks, and leaving before the shift is over will be considered as occurrences.
 For every three (3) events of tardiness, it will be counted as one (1)
