Half of the chart on your spread sheet. Of the 60 charts I reviewed, from January and February, all were outpatient accounts: ED, OBS, Day Surgery, and Outpatient. 95% were ED or OBS patients with the PT/PTT order originating in the ED.

I worked with the ED a few months back and had the PT/PTT orders unchecked in the ED order sets where it was clinically appropriate. This work was completed and is in OneChart.

This however does not stop the provider in the ED from ordering a PT/PTT when they use the order set and they believe it is clinically appropriate. I did also check to see if there were favorites saved with the PT/PTT pre-checked, and none of the ordering providers had favorites with those settings.

From my perspective, this appears to be a practice pattern in the ED for certain patient populations. The order sets below are the most commonly used, and there was also a subset of providers who used single order entry.
