SOCIO-DEMOGRAPHIC SHEET:
Patient’s socio-demographic data will contain identification data (name, age, sex, address, contact no) and socio-economic data (occupation, marital status). It will be collected in the case record form (Appendix 1) by simple questionnaires.
CLINICAL DATA SHEET:
Patient’s clinical details will be recorded in case record form (Appendix 1) in respect to clinical examination details, laboratory investigation reports (hematological, biochemical and radiological data) and treatment details. This data will be collected from the patient’s clinical records and treatment chart.
