PERSONAL PROFILE:
Mark is a 58 year old man who resides in Hatfield, independently. Mark graduated from North Penn high School. Mark is diagnosed with developmental and intellectual disabilities, as well as depression, OCD and anxiety. His last employment was at Impact in Montgomeryville. He worked there for about 4 years as a janitor. Mark had a job coach at Impact and left due to issues with a coworker after the job coach faded off. Mark has difficulty with reading and skills that involve using money. Mark gets frustrated when people around him don’t understand disabilities. Mark has his driver’s license as well as a car and the ability to drive himself to and from work.

GOALS/VOCATIONAL PREFERENCES:
Mark’s preferred employment is working in a grocery store stocking or bagging. Mark has expressed some interest in working at a casino as his dream job. After the assessment, he stated that he would like to work around people. The only task he would not want to perform in a job setting is working with computer. He did not feel confident of comfortable with this task. Mark’s employment opinions seem fairly open and he has motivation to work and obtain skills to improve his quality of life.

He showed motivation to work to complete the tasks asked of him. When asked to return various items he commented that he was taking too long and an employer would not be happy with the speed. He seemed open to each task and informed the job coach when he wasn’t comfortable with a task. He commented on returning the items as well as using the computer; both he was not comfortable or confident with. Mark was dressed appropriately for each day of the assessment, he dressed differently for a grocery store and an office setting.
