Sam’s first goal that she’d like to address is the issue of habitually partying. Social worker and Sam have agreed that what’s attainable is rather than attending a party everyday of weekend (Friday through Sunday) she will attempt to cut back to two days of the weekend only. Client will begin to work towards this goal starting next weekend and will practice this for four weeks. Client will keep a log of the days she went to a party for measurability. After four weeks client and social worker will evaluate progress and if goal has been met then Sam will attempt to reduce her partying days from two days of the weekend to one day only. At that point she will practice the one day a week for another following four weeks. Sam has mentioned she really enjoys reading comic books so it was agreed that when she felt the urge to leave to a party she will steer away from those thoughts by turning to her comic books.
Following the goal of partying less Sam would like to address her drug abuse issue. Client and social worker have agreed

Sam will find social support in AA meetings that she will attend twice a week for three months (can be longer if clients desires). Social worker will also have access to sign in sheets provided at AA meetings to follow up on attendance. Sam will be joining her high school comic book club in an attempt to make new friends. She will join this club within four weeks of being discharged from rehab. Client also will begin to build a relationship with her family and become more integrated in activities. She mentions her family has game night every Friday; therefore she will start off by spending more time with her family by attending game nights. Client has allowed us to have her mother and stepfather speak to social worker to get a perspective on Sam’s reintegration to the family as well as asking if she’s been attending game
