Social Work Case Conceptualization

The patient remains aggressive and violent to himself. He always insisting that he really wants to hit his face and body as told by the voices in his head. He stated that "I will do anything for me to die, I want to die, The voices in my head are screaming to me, they are just waiting for me to do it ". He still has poor judgment, insight and impulse control with severe internal stimuli. He still cannot plan for personal safety. He still helpless, hopeless and worthless with disorganized thought process. He still refuses to contract for safety, still a danger to self. He still needs an inpatient psychiatric hospital level of care.
